Description of Approach

DESCRIPTION OF APPROACH OF CONTRACT COMMUNICATIONS COMPANY WHILE WORKING ON UNDERGROUND PROJECTS.
******************************************************

We will preform a pre-contruction walk off.The purpose of this is to determine if there are any unusual situations. If any changes to the work were necessary,we would contact the customer at this point and resolve the issue.

Any changes would be documented on the Master Print with the name of the person giving permission for any change and the date and time.at this time any changes form,as required by the customer, would be filled out and submitted by the immediately. We would also follow up with a written memo confirming the change to the person who granted the change. We feel that timely proper documentation avoids later problems.

The next step would be to call Appropriate One Call locating facility. and any other utilities not included in one call, Schedule the work to be preformed. If needed, allow locators additional time depending on the amount of existing underground facilities.Door tag property owners, informing them of the work we are doing, and requesting them to contact us if they have any underground facilities on their property(invisible dog fence, drain lines, electric lines etc)Identify on the master print any private facilities that property owners have indicated to us. These prints will always be on job site to assist in clarifying any problems that may arise.

We will access each area and determine the potions of the construction that can be plowed, Directional bored, trenched,or hand dug. at this time we would contact the appropriate customer Representative, to inform them of the work we will be doing in the area.and follow any and all requirements by the customer and or the town, county or state,as far as notification and proper paperwork to be completed prior to work.Including exact address locations, type of work to be performed in each area,Foreman names, and contact numbers for each crew.

Our crews would be split up so that they would be Boring, hand digging, trenching or plowing where needed. If equipment is to be used in front of a home, then we will attempt to contact the homeowner and inform them of the work we are doing and answer any questions or concerns they may have. Our restoration crew will follow directly behind the direct bury crews so that we can provide same day restoration. Normally we provide black dirt and seed restoration. In areas where the property had sod, we will restore the sod. We will door tag the customer at the end of the job to let them know that our construction is complete and they should water the restored area occasionally. Our objective is not to leave any trench line open at the end of day

At the end of every day we will document the work completed by each crew with the proper paper work as designated by the customer and any government agency required to be notified. The next business morning we will report to the appropriate customer representative ,all the work completed and material used, and supply proper paperwork.

Our Splicing crews will follow the direct bury crews within 48 hour. The splicers will document all signal readings, and the work performed at each location, including addresses.

All material, machinery, and trucks will be stored in an appropriate area. We will not store any items on private property without  permission. All machines and material will be stored at a yard supplied by Contract Communications Co. Upon completion of the project, any remaining material, machinery, and trucks will be moved from the job site.

A construction supervisor will walk off the project upon completion of the work taking footage, checking the quality of the work, and splicing. The work will be documented on Asbuilts immediately.

Throughout the construction process, all fields, office conversations and communications between Contract Communications Co. and the customer or Contract Communications and the property owner will be logged. We will log all inquiries, complaints, and compliments by property address with names if possible. Any damaged utilities will be logged by property address with the appropriate dig number, and a damage report will be completed. Any damages will be called to the appropriate utility so the necessary repair can be made. We will contact the appropriate customer representative to notify of damage. We will also contact the state locating service to verify their original dig number. Pictures and/or video will be taken and kept on file. This file will be accessible to anyone who desires.
